Project IMPACT is a partnership between The Youth Council and the Integrated Delivery Network through Southern NH Health and the Nashua School District that provides screening and intervention services for youth who are struggling with stressors that may lead or have led them to develop challenges with positive behavioral health.

- Collaborate with parents/ guardians, school personnel, primary care providers and the youth
- Recommend an array of support and services
- Develop an Action Plan based on individualized needs and goals
- Check-in with the youth to monitor progress and support his/her efforts
This program is a free service offered to you through the Project IMPACT program and is hosted virtually.
